We offer summer camp for children ages 3 – entering 1st grade.
Must be potty-trained and have Physical and Immunization records on file.

ALL campers need to have the following forms turned in with the Registration Form:

• School Entry Health Form

• Immunization Record

(the above forms need to be on State of Florida forms from your pediatrician)

• Emergency Contact Form (Notarized)
